Myocardial infarctions are commonly caused by the occlusion of a coronary artery. This occlusion then leads to ischemia and necrosis of myocardial tissue. The pathophysiological sequence following an MI can lead to developing of left ventricular failure. During an MI, myocardial tissue creates irreversible damage due to ischemia. Scar tissue develops after the ischemia, which compromises the heart’s ability to contract. The scar tissue then leads to impaired systolic function. Initially, the heart tries to compensate such as increased heart rate and the activation of the sympathetic nervous system. However, these mechanisms can lead to further myocardial stress. As LV dysfunction progresses, the heart may fail to adequately eject blood. Heart failure then can cause pulmonary congestion and edema. In summary, the pathophysiological progression from MI to LV failure involves myocardial damage, compensatory responses, and ultimately remodeling leading to impaired cardiac function and heart failure.
Several factors that can affect the severity of LV failure a following an MI. The size and location of the infarcted area determine the degree of myocardial dysfunction and LV failure. Early therapy (e.g., thrombolytics, percutaneous coronary intervention) can limit myocardial damage and improve LV function. Comorbidities such as hypertension, diabetes, and chronic kidney disease can exacerbate LV dysfunction post-MI. Older age and a family history of cardiovascular disease may contribute to poorer outcomes and increased severity of LV failure. Compliance with medications such as beta-blockers, ACE inhibitors, and diuretics.
After reviewing the CDC article and video regarding blood clots. The diagnosis of DVT or PE requires special tests that can only be performed by a doctor.  Medication is used to treat DVT. Compression stockings (also called graduated compression stockings) are sometimes recommended to prevent DVT and relieve pain and swelling. Immediate medical attention is necessary to treat PE. In cases of severe, life-threatening PE, there are medicines called thrombolytics that can dissolve the clot. Other medicines, called anticoagulants, may be prescribed to prevent more clots from forming. Some people may need to be on medication long-term to prevent future blood clots.

Mrs. Lyons is a 57-year-old African American female who recently developed headaches and had an elevated blood pressure reading at her work health fair. Her blood pressure was taken at the health fair, and the reading was 168/99. She has returned to the drugstore three times to take her blood pressure. The readings have been: 145/90, 150/89, 140/88. At the health fair, she was told her BMI is elevated. Her cholesterol levels were also done at the health fair, and the results were: total cholesterol level of 250, LDL 138, HDL 48, and Triglycerides 170.

In this Week 5 assignment, you will take the next step in the nursing process when you plan a health promotion intervention which addresses your community health needs diagnosis.
You have completed a community assessment and analyzed and diagnosed community concerns and/or conditions. It is now time to take the next step in the nursing process and develop a plan to address the community health diagnosis. You will develop a goal statement, and objectives and strategies for reaching that goal. You will identify a Health Promotion Model (presented in week 1) to help design those objectives and strategies.
Addressing a community need is never done by just one nurse alone. A group of dedicated stakeholders and interprofessional health promoters and problem solvers work together to develop a plan they will be involved in implementing. In this assignment, you will identify the stakeholders who would be involved in implementing the Health Promotion Plan.
Objectives:
For this assignment, you will:
Review your community health diagnosis and its etiologies and identify a focus of a Health Promotion Intervention.
Develop a plan for a Health Promotion Intervention which includes use of a Health Promotion or Nursing Model and identification of stakeholders.
Steps
You will review the community health diagnosis you developed and identify the focus of your health promotion project.
To prepare for this assignment, under Materials above, refer to How to Write a Plan for tips on how to write goal, objective, and strategy statements.
In a written document, you will:
State the problem identified as written in Week 4 including the evidence and conditions related to the problem. Review instructor feedback from week 4 to ensure the problem can be addressed within the scope of this assignment.
State who the focus (aggregate) of the health promotion will be, for example, your whole community, a group within it, or select individuals.
State a goal for your health promotion project.
The goal must be related to the diagnosis and within the scope of a CHN (see the Tips section below for examples.)
List at least ONE objective for addressing the goal.
Objectives would be ways in which you can reach the goal. There might be far more than just one idea for how to reach the goal, but you are only required to list one. The example paper (linked above under Materials) gives you an indication of how a student might implement only a portion of the full plan you create.
Identify a Health Promotion Model (Week 1) you will use to develop your plan.
Using the language of the selected Health Promotion Model, identify two strategies for addressing the one objective you have listed. Strategies must be within the scope of a CHN.
Identify stakeholders who would be involved in implementing each strategy.
AGAIN: Ensure that your goals/objectives/strategies are within the scope of a community health nurse and “doable.”
Remember that not only is this a paper you are turning in for a school assignment, but it is also a “documentation” of your planning efforts. Just as you document your plan in your acute care nursing practice, Health promotion nurses must document their plans as well.
5. Your Planning section will eventually be added to your Assessment and Windshield Survey, and your Diagnosis & Analysis in week 7 to create a complied document. Each section will have its own introduction, but you will use the title page and running head that you created in Weeks 2 & 3 and 4. The heading for this section should be:”Planning.” Use the template in “Materials Needed” below to create this component of the process paper.
