To develop the Goals and Objectives for the Community Teaching project, follow the following steps:
The goal or aim of your project is a broad statement of what you want to achieve.
Analyze the problem or the issue.
Find out what is causing concern and break the issue into its components: “Patients with Diabetes.”
Identified the Stakeholders (‘key players’ or main stakeholders and invited them to be part of the project) “Talk to the Diabetes educator.”
Know your target group (a group, for example, adolescents with Type I DM)
Understand the characteristics of your target group, “recently diagnosed diabetes patient,” “pre-menopausal women at risk for diabetes.”
Determine the result you want from your community education project. Define this regarding a goal, educational objectives, and desired outcomes.
Read the following article, Goal Setting and Action Planning for Health Behavior ChangeLinks to an external site..
Objectives
Define your educational objectives regarding the knowledge, skills, values, beliefs, and practices you want the target group to adopt to achieve the goal.
Educational objectives:
At the end of the project, participants will:
Verbalize increased knowledge and understanding of the diabetes disease process.
Be able to identify the signs and symptoms of hyper and hypoglycemia.
Be able to identify solutions to the environmental impacts.
develop an appreciation for a glycemic diet and the importance of monitoring blood glucose.
Use of the American Diabetes Association guidelines for blood glucose control.
Design your educational project (based on evidence)
Make an action plan and implement it.
Evaluate the objectives by testing or surveying the knowledge and attitudes of the participants concerning the purposes.
Consider This:
SMART:
Specific: Clearly state the issue, the target group, and the time and place of the activity.
Measurable: Be clear about what will be changed and by how much. Setting this clearly at the start makes it easier to evaluate. Example of a measurable goal: “Within the next six weeks, I will develop a health literacy program for at-risk of amputation diabetic patients in the community. I will survey at the end of the program. 80% of participants will answer at least five questions correctly in the survey.”
Achievable: Be realistic about what the project achieves regarding the scale/scope of what is being done, time, and resources available.
Relevant: Objectives need to relate to and be relevant to the goals. Remember, objectives are the building blocks/steps toward meeting the goals.
Time Specific: Be clear in the objectives about the timeframe in which the program/activities, as well as expected changes, will take place
Remember that the rest of the course will be used to accomplish your goal by having a clear goal.
Goal statements “identify” the specific target group and provide the “what” information as distinct from the “how” the goal will be achieved or when it will come about.
Objectives are the building blocks or steps toward achieving a program’s goals. Objectives are specific and concise statements stating who will make what change, by how much, where, and when.

The SOAP note is a commonly used narrative transcription of a client’s health data. It can be used to identify and explain the client’s problem-oriented complaint and comprehensive history. For this assignment, utilize the attached Word document to record a comprehensive history and client examination in a narrative format.
Subjective Data: What the client or family members tell you about the client’s signs and symptoms and the reason for seeking healthcare. Typically, this is documented by quoting the actual words said.
Past Medical History is subjective data the nurse collects about any past medical history.
A review of systems is subjective data collected as a list of the body systems obtained through a series of questions to identify signs and/or symptoms the client may be experiencing.
Objective Data: Factual, measurable clinical findings such as LOC, vital signs, and clinical findings on assessment.
Assessment: Evaluating clinical findings through Inspection, Palpation, Percussion, and Auscultation. All information obtained is documented in the client’s history and pathophysiology.
Plan: Short-term and long-term goals and strategies that will be used to relieve the client’s problems.
Complete the following template and submit documentation for the comprehensive health assessment.

My Clinical Scholarly Project
My clinical Scholarly project focuses on the prevalence of depression among adolescents in the American society. This explores the issue of adolescents’ depression in American society, highlighting its impact on well-being, academic performance, social connections, and the risk of substance misuse or suicidal ideation. The study emphasizes the need for specialized therapies aligned with the preferences of today’s tech-savvy and socially aware youth. The incidence and prevalence of adolescent depression present a worrisome trend, emphasizing the need for urgent intervention.

 1. Sustainability is defined by Merriam-Webster (n.d.) as the “harvesting or use of a resource so it is not depleted or permanently damaged”. The United Nations Brundtland Commission (1987) identified sustainability as “meeting the needs of the present without compromising the ability of future generations to meet their own needs” (as cited in United Nations, n.d., para. 1). In my nursing practice, the thing that stands out to me most regarding sustainability is the use of antibiotics.
John Hopkins Medicine (n.d.) highlights that when an antibiotic is given unnecessarily or improperly it increases the chance of bacteria becoming antibiotic-resistant. Also, there are some antibiotic-resistant bacteria present in the infection which is why some infections last longer or the antibiotics do not seem to be as effective (Johns Hopkins Medicine, n.d.). Therefore, I always advocate for narrow-spectrum antibiotics that cover the target bacteria. I also profusely educate my patients on the proper use of antibiotics and to complete the entire regimen.
2. Concepts- Decision Support
Decision support in healthcare informatics refers to the computerized applications that improve the decision-making structure within healthcare facilities. Decision-making informatics rely heavily on organizational data to make the recommended decisions for healthcare practice (Vyas et al., 2022). Data systems entail the connectivity of data collection, recording, point-to-point relay, analytics, and predictive software that improves the accuracy of healthcare avenues’ management. Decision support advances healthcare research and innovation by identifying key trends and patterns in healthcare. Computerized decision-making in healthcare is critical to attaining scalability objectives and reducing the chances of medical errors. It enables informed decisions for medication, resources, and healthcare intervention models (Sharma et al., 2022). Decision support systems in healthcare are critical in policy formulation, such as allocating healthcare resources in departments, including nurses’ scheduling and improving toiletry interventions for acute care geriatric patients.
The decision support informatics construct in healthcare entails three definitive aspects: the software, the database, and the user interface. Computerized software is the functional aspect of a decision support system that defines the programmed set of instructions that performs specific tasks according to the predefined desired outcomes (Vyas et al., 2022). Decision support software includes analytics software that analyzes patients’ data, such as medication history and the prevalence of health risks in society. The computerized database refers to the central storage of healthcare information. Healthcare providers adopt centralized databases accessible to specific caregivers to ensure coordination in care interventions. For instance, the French addictovigilance network improves prescription medication screening to reduce the prevalence of prescription medication abuse (Sharma et al., 2022). The computerized interface refers to the access points of the key personnel in healthcare. The user interface provides understandable language and input/output prompts that enable healthcare providers, patients, and stakeholders to coordinate care implementation objectives.
At my healthcare facility, decision support informatics are implemented through the Care Cloud advanced analytics suite. This software improves data analysis in the healthcare facility by collecting data and providing real-time and prospected data reports. The software suite entails remote surveillance of patient health status through connected applications such as MySugr, which monitors HbA1c, and the Apple Watch (Sharma et al., 2022). This interconnectivity complements remote healthcare delivery through telehealth services by offering real-time connectivity with patients to enable decision support. The care cloud advanced analytics suite improves assistant nurses’ monitoring of patient’s health and identification of variations and potential indicators for health deterioration. Further, this software identifies patients at a higher risk for readmissions, higher risk for adverse medical reactions, and potential noncompliance to medication regimes (Vyas et al., 2022).   This software is a vital tool for supporting healthcare decisions.
