You are the Chair of the Ethics Committee at the hospital where you work. An inpatient makes a request of his care team that makes them uncomfortable, and they contact you for advice. You convene the Ethics Committee to discuss the situation and must provide the care team guidance on short notice. Here is the patient’s situation and request.
The patient is an 87-year old insulin dependent diabetic. He was admitted for altered mental status related to uncontrolled blood sugar levels. Once stable, he reports that he wishes to stop taking insulin to control his blood sugar, and declines the nutritional advice of his dietitian. His physician tells the patient that these actions will prematurely end his life. The patient has no terminal condition(s) and is of sound mind. Some members of the care team describe this patient’s decision as suicidal and refuse to care for him.
What tactic will you use to guide the discussion with the Ethics Committee? Will you take a vote? If so, will you require a unanimous decision or something else? Overview
Blumberg’s Nursing Home (BNH) is a 100-bed Medicare and Medicaid-certified facility in suburban Philadelphia, PA. The administrator recently terminated the facility’s DON, receptionist, and dietary aid for inappropriate conduct. They had all been involved in purchasing and using illegal substances on facility grounds. The facility staff and many of the more lucid residents have found out about the incident.
The charge nurse has been appointed interim DON. The human resource manager has sent a memo to all staff informing them of a mandatory staff meeting to discuss BNH’s code of ethics and to provide additional training on workplace culture. Because federal law requires that there be at least one registered nurse on duty for a minimum of eight hours a day, seven days a week and that the DON must be a registered nurse (RN), which the interim DON is not – BNH is out of compliance.

Bella’s mother sought treatment due to increasing disruptive behaviors over the past year, including non-compliance, physical aggression toward peers, and frequent behavioral meltdowns which resembled the temper tantrums of a much younger child. Tantrums included screaming, yelling, slamming doors, and crying. Bella and her mother both noted that it was difficult for Bella to “move on” when something angered her. She also noted that Bella had an underlying irritable mood, manifesting as Bella appearing “cranky” most of the time and the family feeling they needed to “walk on eggshells” to avoid upset. At school, at least one phone call home per week was being placed due to Bella’s refusal to comply or sometimes to even speak to her teacher for days at a time. Bella and her mother noted that Bella was generally well-liked by peers and teachers, given that she was hardworking and funny, yet her current disruptive behaviors were causing significant interference in making new friends and meeting academic goals.
