Project Implementation Plan

The proposed change project deals with type 2 diabetes, aiming to improve patients’ health and knowledge about management. The diabetes self-management and education and support (DSMES) toolkit is the main framework and source of information for this particular intervention (Centers for Disease Control and Prevention, 2021). During the first and second weeks, the selected participants will be educated using the DSMES toolkit and how to record their use of the information and change in blood glucose. For this step, a milestone is completing the educational process for all participants and providing all necessary materials. In weeks 3-5, the participants will record their data, provide feedback, and ask questions if they occur. The 6th week is devoted to data collection and structuring – the milestone is a set of usable data systematized for quantitative analysis. During week 7, the gathered information will be analyzed, and the milestone will be an evidence-based answer to the PICOT question. The final week’s milestone is completing the discussion portion of the PICOT project based on the acquired data.

Project Success

The project’s success depends on several factors and is determined by how the main question is answered. In the current PICOT project, the question is whether the DSMES improves adult patients’ fasting glucose levels compared to usual treatment. In connection to the project’s goals, a successful conclusion would show a clear recommendation or a lack of it for using DSMES for patients with type 2 diabetes (Burd et al., 2020). The process-based success would be presenting data analysis based on sufficiently gathered data.

Personal Role

During the planning process, the role of the project manager is to complete the cost-effectiveness analysis, create the budget, and adjust it based on the feedback of organization leaders. Finding the sources for allocating resources and establishing connections with team members is another vital part of this role (Bayram et al., 2022). During the implementation phase, the manager should oversee the resources’ use and control the activities to stay within the budget. If changes occur, it is necessary to alert other members of the team and organization leaders.
