Sarah is an Early Intervention ABA Provider. She has a very heavy caseload and travels by car to multiple sessions each day.
Since she is in a rush between sessions she leaves a file for all of her session notes in the back seat. She frequently leaves her car unlocked so she can access toys and materials she needs in middle of a session.
Esther is Sarah’s client who receives 10 hours of ABA EI services per week. Sarah has an excellent rapport with Esther and her parents, and Esther has made significant progress in the past few sessions.
At the most recent session, Sarah mistakenly handed a parent a session note containing another child’s name and diagnosis code. She quickly replaced it with a blank session note. Esther’s mother told Sarah not to worry.
Sarah is having an issue regarding her client Esther’s feeding program. Esther is not making progress and Sarah decides that it would be appropriate to consult with the speech therapist on an appropriate intervention at this point. She calls the speech therapist without consulting the parents first since she assumes this is in the child’s best interest.
Sarah emails Esther’s IFSP using her private email account without a password to the provider who will be Esther’s SEIT after she ages out of EI to ensure that the SEIT is updated on all of Esther’s IFSP programs.
